Before filling out this application form, it is very important for you to read the Grant Guidelines to be sure your proposal qualifies for consideration. In order to receive consideration by the Foundations Board of Directors, applications must be postmarked by the deadline. Application deadlines are March 1 and September 1 of each year.
Please type the answer to each question concisely within the space indicated. Do not extend your responses to extra sheets of paper. An application form submitted with answers expanded beyond the space indicated will not be considered.
Submit ONE (1) original and TWO (2) copies of the completed application form. Attach 1 copy of a brief cover letter and 1 copy of the material requested for attachments.

Adobe Acrobat Reader PDF Version (275K)
The application form is designed to be filled-out on your computer and printed-out on your printer, then mailed to the Foundation. The form is an Adobe Acrobat Portable Document Format or PDF.
To download and fill out a PDF you need Adobe Acrobat Reader installed on your computer. Adobe Acrobat Reader is a free download from Adobe web site. If you do no already have Acrobat Reader, use the button provided here to download and install Reader from the Adobe web site.
With Acrobat Reader installed, download the application form and it will open within Reader, ready for you to fill in the form fields and then print. Press the Tab key to advance from field to field, or just mouse-click in a text field.
Reader does NOT allow you to modify the Application form itself, nor can you save data entered into the form. You can save the form, but only without data. You might try printing out the form to get a sense of the size of the boxes before you begin entering your data.
Microsoft Word Version
Unlike the Adobe Acrobat Reader PDF version of the application, you can save, re-open, and edit the Word version of the application.
Please be brief and limit the application to a maximum of four pages. Your word processor version of the application should be as similar as possible to the on-line Adobe Acrobat Reader version of the application.
Here are some Microsoft Word versions of the application form that you can download and use. If you use one of these documents, we suggest that before you begin entering your data, that you first print it out so you can get a sense of the size of the spaces allowed for your answers. Applications that exceed four pages will not be considered.
